Purpose-To investigate the prognostic significance of Injury Severity Score(ISS) correlated to the height of fall in free fall patients. Patient and methods-The medical records of 179 free fall patients who were brought alive to the emergency department of Gyeongsang National University Hospital between January 2000 and January 2004 were analyzed. Patients were analyzed and evaluated in terms of the age, sex, rate of admission, hospital stay, injured parts, severity, rate of operation, the parts of direct infliction from falling, and whether they were drunk at the time by retrospective review of the medical record. Severity of each case was measured by the ISS. Patients were categorized into 4 groups according to the falling heights. The data were statistically analyzed with SPSS ver 10.0. Result-The admission rate for the groups with falls of less than 3 stories was significantly lower than the subgroups with higher heights of falls (70.7% vs 100%, p<0.05). These two groups showed statistically significant difference in the mean hospital stay (17.11 ± 24.88 vs 56.73 ± 49.21, p<0.05), in the rate of operation (30.6% vs 53.8%, p<0.05), and in the mean ISS (6.86 ± 4.97 vs 13.96 ± 9.14, p<0.05). In the correlation analysis, the ISS and the mean hospital stay showed the highest correlation with the correlation coefficient of 0.666. Conclusion-In this retrospective analysis of 179 free fall patients, the author evaluated the prognostic factors affecting the outcomes of the free fall patients. The patients with the falling height of 3 stories(≒ 6 m) or higher showed statistically significant higher rates of admission, longer duration of hospital stay, higher ISS, and higher operation rates. And the most accurate factor in predicting the length of hospital stay was the ISS.
