Objective To assess the inter-tester and test-retest reliability and validity of PACS(picture archiving and communication system) for measuring of leg length discrepancy Method The study included 30 healthy adults volunteers among medical school students and physical therapist without neurological abnormality. Both leg lengths were measured once with tape ruler from femoral head to end of distal tibia for conventional method and twice with PACS at both lower extremities. Leg length discrepancy was defined as difference between right and left leg lengths. The inter-tester and test-retest reliability of conventional method and PACS were calculated as Pearson correlation coefficient by SPSS. The validity of PACS was also calculated as Pearson correlation coefficient by SPSS. Results 1. The relationship between PACS1 and conventional radiographic method The correlation coefficient of right leg length, left leg length and leg length discrepancy were 0.996, 0.996, 0.606 respectively in examiner A and 0.996, 0.992, 0.624 respectively in examiner B. All the coefficient were statistically significant at 0.01. 2. The relationship between PACS1 and PACS1 The correlation coefficient of right leg length, left leg length and leg length discrepancy were 0.997, 0.998, 0.747 respectively in examiner A and 0.998, 0.995, 0.790 respectively in examiner B. All the coefficient were statistically significant at 0.01. 3. The relationship between examiner A and examiner B The correlation coefficient of right leg length, left leg length and leg length discrepancy were 0.999, 0.999, 0.864 respectively at conventional radiographic method, 0.999, 0.998, 0.709 respectively at PACS1 and 0.999, 1.000, 0.784 at PACS2. All the coefficient were statistically significant at 0.01. 4. Leg length discrepancy difference at PACS1 and PACS2 Leg length discrepancy difference measured by PACS1 and PACS2 was 0.220.23 cm(SD) and 5 cases were more than 5 mm but non of them was more than 10 mm in examiner 1. It was 0.190.14 cm(SD) and only 1 case was more than 10 mm in examiner 2. Conclusion PACS is clinically reliable and valid tool to measure leg length and large amount of leg length discrepancy more than 10 mm.
